We offer this service to people 18 years old or older who live in their own homes with needed support service and training in the community, so they can maintain their quality of life. These services can be done in person or remotely. The broad service categories for providing support and training to help a person (s) to live independently in the community include the following:
- Community participation
- Health, safety, and wellness
- Household management
- Adaptive skills
For the support services, our staff will provide direct supervision, cueing, maintenance, guidance, instruction, incidental assistance with activities of daily living (ADLs), as well as assistance with coordination of community living activities. For the training services, our staff will provide support with instructional services through which the person(s) will receive direct training from staff on community living skills identified during a person’s assessment (e.g., MnCHOICES Assessment). The training may include skill-building to acquire, retain, and improve the person’s quality of life in the community.
